Providing housing assistance and supportive services to homeless individuals


Metro Lutheran Ministry provides permanent supportive housing services to high-need, chronically homeless individuals in Missouri and Kansas through a program called Grace Homes.

MO-604 Continuum of Care, serving Jackson and Wyandotte Counties

Permanent Supportive Housing is a program that provides long-term permanent rental housing with supportive services to homeless individuals who have disabling conditions including severe and persistent mental illness, substance use disorders, physical, developmental, or other behavioral disabilities.

Participants are referred by the local Continuum of Care's HUD approved Coordinated Entry by-name list.
