Project CARE is an intensive strength based case management program of Metro Lutheran Ministry. The goal of the program is to assist homeless families and single individuals to:
1) Obtain permanent housing
2) Increase their income
3) Achieve greater self determination
Four case managers and a job assistant/life skill instructor comprise the Project CARE team. The case managers focus on client strengths as they assist participants living in shelters, parks and abandoned buildings identify ways to overcome barriers to employment and permanent housing. Participation in Life skills is essential in helping the client achieve their personal goals.
Individuals who successfully obtain housing can receive rent assistance and other services such as furniture and groceries to help stabilize their household.
Follow up case management can continue up to 6 months after an individual finds housing to ensure stabilization and success in the community.
